Under the Americans with Disabilities Act (ADA) and Section 504 of the Rehabilitation Act of 1973, individuals with disabilities are guaranteed certain protections and rights to equal access to programs and services. In order to access these rights, it is the policy of the University of Northern Iowa (UNI) for students with disabilities to register with the office of Student Disability Services (SDS) in order to determine eligibility for academic accommodations.
Accommodations for students are determined by SDS staff on a case-by-case basis after reviewing both the student self-report and supporting documentation. Upon acceptance and decision to attend UNI, students with disabilities should complete the following registration procedure in order to receive accommodations and services through SDS:
Step One: Complete and submit a Request for Services Form
The Request for Services Form provides SDS with an introduction to the student’s disability needs and academic accommodation history.
Step Two: Submit documentation in support of the request for academic accommodations
Documentation provides SDS with additional information regarding a student’s disability status and academic accommodation history, and provides additional support for accommodation requests.

What Happens Next?
SDS will review the information submitted and contact the student to schedule an initial accommodation appointment.
Student meets with SDS to discuss academic accommodation needs and to determine eligibility for services.
During this meeting, the student will meet one-on-one with an SDS staff member to discuss their academic accommodation history and disability needs. At the end of this meeting, the SDS staff member will usually determine the student’s eligibility for accommodations. If additional documentation is needed, SDS will inform the student of this requirement at the time of the meeting.
If the student is approved for accommodations, the student and SDS staff member will complete the necessary paperwork to inform the student’s instructors of her/his academic accommodations.
