Each team will have final reports
written and submitted by December 1, 1998.

1. Provide evidence of a collaborative effort
among the team members:
a. Provide the team leader's name and school district.
b. Provide each team member's name, school district and the role
and responsibilities of each team member.
c. Tell us if any of the original team members dropped out of the
project and why.
2. Provide evidence of
administrative support for K-12 team members involved in this project.
Discuss the role of K-12 administrators of the schools involved in this project. Tell
us how the administrators supported the project and the educators involved in the project.
3. Provide evidence of an
exemplary project design:
a. Tell us about the
audience for the final product:
- Grade level.
- Content/subject area.
b. Tell us about the
final product:
- What is the WWW-based curriculum?
- Student activities included with the WWW site.
- URL of the WWW site.
c. Discuss the intended impact on students and teachers who used
the WWW-based curriculum product.
d. Outline how your team met all steps in the Instructional Design
(ID) process:
Step 1. Needs assessment
Step 2. Identify learning outcomes (goals/objectives)
Step 3. Determine learning medium
Step 4. Plan and script the final product
Step 5. Produce materials
Step 6. Evaluation & Assessment
e. Outline the timeline for your project. Include such things as
planning sessions, work sessions, data collection time, WWW site creation time, WWW pages
upload procedures,WWW site evaluation plans, WWW site testing plans, piloting the WWW
site, revising the WWW pages, instructional materials creation time, curriculum
implementation plans, and final report writing time.
f. Provide your final budget, along with a short description
of any resources purchased. Keep in mind that use of the funds must be directly related to
the project, to the development of WWW-based curriculum materials, and to learner
outcomes. The funds do not represent an award or prize, but instead are grant program
funds to be used for the creation of WWW-based curriculum materials.
Grant funds may be used for:
- software and peripherals for the project.
- substitute teachers.
- travel for team collaboration.
- personal expenses related to the project.
- materials development.
- hiring experts for high-end web and graphics applications.
- stipends for team members are allowable but for no more
than 30% of the total funds.
Grant funds may not be used for:
- computers.
- file servers.
- web servers.
- workshops.
- previously purchased items.
Itemize your budget expenditures, totalling $10,000. Keep in mind that
one-half of the money should have been given to your team immediately following the summer
workshop and the other half will be given to your team at the conclusion of the project,
pending approval by the Planning and Implementation Team of the project, including the
final product and the budget expenditures.
4. Describe any problems encountered while developing,
coordinating or implementing the WWW-based curriculum.
5. Explain how the WWW-based curriculum coordinated with each
school's technology plan, school improvement plan, building goals and/or computer training
program.
6. Be sure to complete and return any evaluation forms sent to
you by ISU's Technology Research and Evaluation Group.
Final Report Requirements - Addendum
To meet the research and evaluation requirements of the Iowa State University research
group, the following information should be added to your final reports.
1. Provide evidence of a collaborative effort among the team
members:
d. Provide the names of all schools that participated in your project
and who utilized your final product, if different than the schools listed in b. for this
question.
2. Provide evidence of administrative support for K-12 team
members involved in this project.
b. Discuss any additional support that you received - ie., from the
AEA, college or university, colleagues, other teachers, students, community.
3. Provide evidence of an exemplary project design:
a. Tell us about the audience for the final product:
- Also include the number of students who utilized the final product; their
demographics; and their reactions to the product.
c. Also discuss the actual impact on students and teachers who
used the WWW-based curriculum product. Discuss changes in skill, attitudes toward the
subject, attitudes toward technology, and changes in behavior.
f. Provide your final budget...........
Also provide the following details:
- substitute teachers - how many; when, where; why.
- travel for team collaboration - when, where, for whom.
- personal expenses related to the project - what, when, where, for whom.
- materials development - describe the materials and the development of the materials;
who developed them; when they were developed.
- hiring experts for high-end web and graphics applications - Who; when; qualifications
and background of the experts.
The following questions are new, additional questions.
7. Discuss how your project changed from the initial proposal to
completion.
8. Discuss how your project team dynamics changed from the
beginning to the conclusion of the project. Include such things as communication styles
and forms, changing of team member roles and responsibilities, and relationships with
others.
9. How will this project continue? Will you expand the site
with additional topics or activities? Will you expand the site into other units, classes,
teachers? How will the expansion be funded?
10. Describe any awards or recognition received for your final
product. Include any requests for permission to use or number of contacts from the WWW.
11. Concluding remarks:
Tell us what this project did for you and your school? What recommendations would you
make to U S WEST for future projects? What recommendations would you make to the project
planning team for future projects?
Please send an original and a copy of your final report to Doreen. She will then send
all copies to the Iowa State University research group. You may also include an electronic
copy of the report on diskette.
