Passport Application at SAC Events

The day of Passport Day (November 20th), passport photos will be taken for free for all attendees!
*If you are a student going on a study abroad program, stop by the SAC anytime during business hours for a free passport photo!
Information for Attendees
Come to the fair to Apply/Renew your U.S. Passport. Below are the items you will need if you are a first time applicant for a U.S. passport and for passport renewal.
For applying for a U.S. passport, you will need the following:
- A completed DS-11 Form, which you may fill out prior to Passport Day and bring with. Otherwise, the Black Hawk Recorder's Office will bring blank forms.
- A certified copy of the applicant's birth certificate. (It should include the applicant's parents' birth names and have a registrar's raised, embossed, impressed, or multicolored seal).
- A photocopy of BOTH the front and back sides of the applicant's driver's license.
- A passport photo (2" x 2" with a white or light gray background).
- Fees for applying for a U.S. passport as follows:
- The passport costs $130.00 and must be paid by check or money order to the U.S. Dept of State.
- Processing fee of $35 for the application paid by check or cash to the Black Hawk County Recorder.
- Black Hawk County Recorder's Office will take photos at a fee of $15.00 during the Fall Study Abroad Fair. During the Passport Day in November, photos are taken for free by the SAC.
For U.S. passport renewal, you will need the following:
- A completed DS-11 Form which you may fill out prior to Passport Day and bring with. Otherwise, the Black Hawk Recorder's Office will bring blank forms.
- If an applicant completes the DS-11 form for renewing a passport, the process and fees are exactly the same.
- If an applicant wants a passport renewal, and they were 15 years or younger when they were issued their passport, they must bring their birth certificate and their passport.
- The applicant will need to bring their old passport.
- Must bring a certified copy of the applicant's birth certificate.
- A photocopy of BOTH the front and back sides of the applicant's driver's license.
- A passport photo (2" x 2" with a white or light gray background).
- Fees for a passport renewal are as follows:

Important Notes: The more items you complete ahead of time, the faster they can service your application. Remember to have a photocopy of BOTH sides of your driver's license.
A certified copy of a birth certificate will have the following:
- Issued by the county or state of birth
- Lists applicant’s full name, date of birth, and place of birth
- Lists parent(s)’ full names
- Has the signature of the county or state registrar
- Has the date filed with registrar's office (must be within one year of birth)
- Has the registrar's raised, embossed, impressed, or multicolored seal of issuing authority
How early can you renew your Passport?
You can renew a current passport at any time. The expiration date for the new passport will be exactly 10 years after the issue date. So, if you apply today, accounting for processing time, your new passport would be valid 10 years from this date.
